Creating a Questionnaire
Only users with firm admin permissions are able to create new questionnaires in Fieldguide. To create a new questionnaire, follow these steps:
Log in to Fieldguide and navigate to the admin settings.
Click on Questionnaires.
Click on + Questionnaire in the upper-right corner.
Give your Questionnaire a Name and (optionally) a Description.
Click Add Questionnaire.
Adding Questions to Questionnaires
After creating a Questionnaire, firm admin users can add questions and content to the Questionnaire.
Step #1: Insert a new question
With a Questionnaire open, click on Add Question in the upper left-hand corner of the questionnaire page.
If your Questionnaire is currently empty, you can alternatively click Add Question in the center of the Questionnaires window.
Step #2: Enter the question text and other details
Enter your question text, any additional details, the category the questionnaire question falls into, and the question type.
If there are no categories yet in the questionnaire, you can create one by typing out the name of the desired category.
There are three types of question types:
Single Select: allows for the selection of one option
Multi-Select: allows for multiple options to be selected
Text: allows for free-form text response
Step #3: Preview your questionnaire
After hitting Add Question, you'll be taken to your question. If you want to change the question type at that point, you can select the dropdown to switch.
To continue building the Questionnaire, add additional questions by clicking + Question in the upper-left hand corner.
To preview a questionnaire, click the "Preview Questionnaire" icon in the upper left-hand corner. This will take you to what an end user's view of the Questionnaire will look like.
